How can I delete my account?

Deleting your Provider Portfolio account permanently removes it and it’s associations from the HealthStream servers. 

 

To delete your account, follow these steps:

  • Open the Provider Portfolio app and log in.
  • Visit the Settings tab.
  • Choose Account from the list of choices.
  • Scroll to the bottom of the Account screen and select Delete My Account.
  • Confirm your decision by tapping Delete.

If you are not able to access the app, you can request account deletion by visiting  https://www.healthstream.com/privacy-request  and completing the form there.  

In certain circumstances, HealthStream is both legally required and permitted to retain some data. To this extent, your data will not be deleted but will only be processed for those purposes.
